Guidance notes

Here are some written instructions on how to add and edit an online form, or if you prefer, there is a video guide at the bottom of this page.

These notes assume you are already logged into the CMS.


To add a new form

  • Visit the page or add a new page to your sitemap ready to add the form block
  • Click on 'Add Content' from the CMS toolbar
  • Click on the ‘form’ block (or search for it) and drag it into position on the page
  • Click on ‘new form’ if adding a new form
  • Click on ‘choose field’ 
  • From the drop-down menu click on the field type required, for example 'Text' or Phone Number'

There will be a form set up in the ‘/demo’ section of your website showing all the field options in use

  • Type in your question (or title of the field the user will complete) and follow the instructions of the field chosen
  • Scroll down and determine whether the question is ‘required’ to be completed before submission of the form is allowed (this will mean the user can not submit the form if they have left the answer empty
  • Click on ‘add question’ in the bottom left of the scroll menu to save this question to the form.

Clicking on ‘Add’ in the right of the menu footer will not ‘add’ the question to the form. This ‘Add’ is for the final saving of the completed or amended form 

  • Once you have added all of your questions/fields, click on the ‘Results’ option in the header tabs

!!Do not click the option to store data on the CMS, this will be unencrypted and fall outside of GDPR!! 

The data will be sent via email to the form recipient.  If you require the data to be saved in the CMS, contact Support who will arrange for data encryption.  We would also advise that you consult with Information Governance.

  • Fill in the submission address of the person or group who are to receive the form outputs
  • Click on ‘Options’ in the header tabs
  • Scroll down and check ‘Yes’ for ReCAPTCHA (this is for security)
  • Update the thank you message which will be displayed when the user clicks on ‘submit’ (if you wish to)
  • Click ‘save’ in the right hand side of the block to save the form

To amend an existing form

  • Visit the page where the form exists
  • Click 'Edit mode' from the CMS toolbar
  • Click on 'edit block' and choose ‘edit’ from the header tabs
  • Click on the ‘pencil’ icon to edit the questions
  • Clicking and holding on the ‘cross’ option will allow you to re-order the questions
  • Clicking on the ‘trash can’ will delete the question
  • Click 'save' in the bottom right footer to save the form
